Vergleichende Studie zur statischen und mobilen Posturografie unter Lärmexposition

  • Motivation dieser Bachelorthesis war die Weiterführung eines Projekts im Rahmen eines Wahlpflichtmoduls zwei Semester zuvor, bei dem mittels des Wii- Balance-Boards die Gleichgewichtsstabilität in Ruhe und Lärm miteinander verglichen wurde. Ziel der jetzigen Arbeit war es, herauszufinden, ob es signifikante Unterschiede der posturalen Kontrolle in Ruhe und in Lärm gibt, gemessen zum einen mit dem VertiGuard®-System, zum anderen mit der Enke-Platte. Des Weiteren sollte verglichen werden, ob sich die Gleichgewichtsstabilität in Lärm genauso gut trainieren lässt wie in Ruhe. Die Probanden wurden audiologisch eingangsuntersucht und in WHO- Gruppen unterteilt, da man ebenfalls herausfinden wollte, ob der individuelle Hörverlust Einfluss auf das Gleichgewicht des Probanden hat. Zu guter Letzt stellte sich die Frage, ob das VertiGuard®-System auf die gleichen Ergebnisse kommt wie die Enke-Platte. An dieser Studie nahmen 17 Probanden teil, die folgende Anforderungen erfüllten: Die Probanden mussten das 60. Lebensjahr vollendet haben. Allgemeine Schwindelsymptomatik, deren Ursache unklar ist, war Voraussetzung. Ein vorhandener Hörverlust war wünschenswert, Normalhörigkeit allerdings kein Ausschlusskriterium. Schwindelsymptomatik, die von pathologischen Erkrankungen wie Morbus Menière ausgeht, war Grund für eine Verweigerung der Teilnahme. Der Lärm, der für einen Teil der Studie benötigt wurde, war eine eigens dafür erstellte Tonspur, die vor Studienanlauf mit einem Kunstkopf von Head Acoustics an einer Bundesstraße und einer stark befahrenen Stadtkreuzung aufgenommen wurde. Die Probanden mussten nach der audiologischen Voruntersuchung einen gSBDT mit dem VertiGuard® durchlaufen und ein Balance-Screening mit der Enke-Platte absolvieren. Bei dem darauffolgenden Termin wurde der gleiche Ablauf nochmals in Lärm durchgeführt. Auf Grund des VertiGuard®-Ergebnisses wurden fünf Trainingstermine vereinbart, bei denen an jedem Trainingstermin fünfmal hintereinander das vorgeschlagene Training des VertiGuard® absolviert wurde. Nach diesem Training wurde ein Abschluss-gSBDT und ein abschließendes Balance-Screening durchgeführt. Anhand dieser erhobenen Daten wurden mittels diverser statistischer Tests die oben erwähnten Hypothesen überprüft. Dabei stellte sich heraus, dass der individuelle Hörverlust bei der Auswertung des Sturzrisikos mit dem VertiGuard® keinen Einfluss hatte. Im Gegensatz dazu ergaben sich bei der Auswertung mit der Enke-Platte aber Unterschiede zwischen den einzelnen WHO-Gruppen, die tendenziell darauf schließen lassen, dass der Hörverlust einen Einfluss haben könnte, der aber auch altersbedingt zustande gekommen sein könnte. Außerdem zeigte sich, dass Lärm keinen Einfluss auf das Gleichgewicht hat. Das Training in Lärm zeigte effektive Erfolge. Dieses Ergebnis konnte sowohl mit dem VertiGuard®, als auch mit der Enke-Platte beobachtet werden.
  • The motivation of this thesis was to continue a project which was elected two semesters ago. During this project the stability of balance was compared in silence and noise with the Wii - Balance Board.The aim of this thesis was to find out whether there are significant differences of postural control in silent and noisy conditions measured with the VertiGuard®-system and the Enke force platform. Furthermore it should be compared whether the balance stability could be trained as well in noisy co nditions as in silence. The subjects were examined audiological and se parated into groups of hearing-loss (WHO) because we would like to find out whether the individual hearing loss influenced the subject’s balance. In the end we wanted to find out whetherthe VertiGuard®-system and the “Enke-Platte” came to the same result considering the individual risk of falling.17 subjects participated in this clinical trial that fulfilled the following requirements: The subjects needed to be older than 60 years. Gene ral indications of sway with ambiguous cause were required. Hearing loss was desirable but a normal hearing was no criterion for exclusion. On the other hand indications of sway which came from medical conditions like Morbus Menièreled to exclusion. The noise which was needed for this examination was a composed audio track that was recorded with the help of an artificial head from Head Acoustics on a main road and a highly travelled intersection in town. The subjects had to run through a gSBDT after the audiological screening with the VertiGuard®-system and a balance screening with the force platform. During the following appointments the subjects had to do the same in noisy conditions. Based on the results of the VertiGuard®, five trainings were arranged where the subjects had to do the proposed training five times in a row. After the five trainings the subjects had to do a closing gSBDT and balance-screening. Based on the collected data the assumed hypotheses were tested with the help of several statistical tests. One result was that the individual hearing loss had no influence on the risk of falling due to the VertiGuard®-system. In contrast to that, the force platform found out differences between the WHO-groups that could indicate an influence, which could as well be due to the age of the subjects. Furthermore it came out that noise had no influence on balance, the training showed actual success. This result could be observed with the VertiGuard® as well as with the force platform.
